Become a Paxis Contractor

In order to commence work with PAXISgroup and receive payment for work performed, the following documents and forms must be received by the Paxis office.

1) New Vendor Setup Form (Use the form below or request a form from invoices@paxisgroup.com)

2) Signed W-9 

3) Certificate of Insurance with the following minimum coverage and requirements:

General Liability - $1,000,000.00 Each Occurrence
General Aggregate* - $2,000,000.00
Product/Completed Operations Aggregate - $2,000,000.00
Personal & Advertising Injury - $1,000,000.00
Worker’s Compensation - $1,000,000.00
Business Auto Liability - $500,000.00 Each Accident
(*subject to a per project general aggregate provision applicable to the project)

Additionally Insured endorsements, per written contract:

Paxis Group, LLC and Owner are included as Additional Insureds as required per written Contract on a Primary and Non-Contributory basis.  Additional Insureds are included as per Additional Insured Endorsement # CG 20 10 10-01 & CG 20 37 10-01 (copies attached).  Waiver of Subrogation status encompasses General Liability, Umbrella Liability and Workers' Compensation.  Thirty (30) days' notice of cancellation. *
* This verbiage should be noted on the COI as a matter of confirmation of proper endorsements only, and should not replace the actual endorsements.
